Case Management for Adults
APC's Targeted Case management
(TCM)
program provides both direct and indirect services to nearly 200 adults
in Milwaukee County who are recovering from mental illness. Case
managers complete an initial psychosocial assessment in an effort to
identify strengths, gaps in services, needs, and deficits. Applicable
services are coordinated and subsequently monitored for efficacy. The
cornerstone of the program is to assist consumers in accessing natural
and professional resources in a manner that promotes true community
integration and an increased quality of life. Targeted Case Management
services are exclusively authorized by the Service Access to Independent
Living (SAIL) program within the Behavioral health Division of
Milwaukee County.
