Case
Management for Youth
Through Wraparound
Milwaukee, APC provides case management services for youth and their
families. Wraparound, which is run by the Milwaukee County Mental Health
Division-Child and Adolescent Services Branch, is only available to
youth and families who have been court ordered to receive services. All
families are assigned a care coordinator who monitors the progress of a
team-developed plan of care. The program consists of an integrated
system of care and services for children and families who have a complex
constellation of needs. Services may include mentoring, tutoring,
counseling, parenting assistance, respite care, crisis mentoring, and a
host of other services.
